General terms

On this page, you can read the general terms and conditions of B4Fysics (individuals). We assume that you comply with these conditions. Our general terms and conditions apply as long as you are under physiotherapeutic treatment and/or participate in a course or training. Participation in activities in the practice is at your own risk.


Article 1. Definitions

  • Practitioner: the person providing healthcare to the patient based on a treatment agreement
  • Treatment Agreement: the agreement between the patient and B4Fysics regarding the provision of individual healthcare
  • Patient: the person receiving individual healthcare based on a treatment agreement
  • Treatment Agreement Confirmation: document in which the patient answers questions regarding their personal situation, physical condition, and personal goals related to the treatment, as well as evaluation and reporting during and after the treatment
  • Treatment Program: a patient-oriented treatment plan aimed at relieving or solving complaints
  • B4Fysics: user of these general terms and conditions, registered with the Chamber of Commerce in Apeldoorn under no. 75149060


Article 2. Applicability

  • These general terms and conditions apply to all treatment agreements between B4Fysics and the patient, unless parties expressly deviate from them in writing
  • These general terms and conditions also apply to any additional agreements, treatment agreements, and services to the patient
  • The behavioral and professional rules of the Royal Dutch Society for Physiotherapy (KNGF) are part of the agreement
  • When you use a service from B4Fysics, you agree to these general terms and conditions and the privacy policy
  • B4Fysics is entitled to change and/or supplement these general terms and conditions if deemed necessary


Article 3. Establishment of Treatment Agreement

  • The practitioner will enter into a treatment agreement with a patient who has expressed the wish to receive help and/or advice in the field of healthcare
  • If the practitioner cannot enter into this agreement, they will explain the reason to the patient and advise them on finding other appropriate care
  • The treatment agreement includes the treatment agreement confirmation, the treatment program, and these general terms and conditions


Article 4. Obligations of the Practitioner

  • The practitioner acts in accordance with Section 7.7.5 of the Dutch Civil Code and the applicable regulations, model arrangements, and/or protocols for their professional group
  • The practitioner is obliged to inform the patient about the proposed examination and/or treatment and developments regarding the examination, treatment, and health condition of the patient
  • The practitioner maintains a file regarding the patient's treatment
  • The practitioner is obliged to maintain confidentiality regarding the patient's treatment, unless the patient has given permission for the disclosure of information


Article 5. Obligations of the Patient

  • The patient is obliged to provide information to the best of their knowledge and cooperate for the benefit of the treatment
  • If the patient is unwilling to follow certain advice from the practitioner, they will discuss this with them
  • The patient will identify themselves at the request of the practitioner
  • When the patient is unable to keep an appointment, they will cancel it in a timely manner, at least 24 hours in advance.


Article 6. Payment Terms

  • These payment terms are inextricably linked to the treatment agreement and other agreements between the patient and B4Fysics
  • The patient is responsible for full payment of services provided to or on behalf of the patient
  • The declaration for insured care is sent directly to the health insurer
  • Uninsured care is charged directly to the patient based on an invoice
  • Appointments must be canceled 24 hours in advance
  • Payment for the practitioner's services is due within fourteen days after the invoice date


Article 7. Quality

  • The practice complies with legal obligations regarding quality
  • All practitioners within the practice are registered in the Central Quality Register of the Royal Dutch Society for Physiotherapy (KNGF) and in the B.I.G. register


Article 8. Liability

  • B4Fysics is not liable for damages resulting from incorrect or incomplete information provided by the patient
  • B4Fysics is not liable for damages resulting from suspension of treatments and/or services due to the patient's failure to pay invoices on time
  • Third parties cannot derive any rights from the content of the treatments and/or services provided.


Article 9. Intellectual Property Rights

  • The patient is not allowed to reproduce, disclose, and/or exploit advice, contracts, or other intellectual property produced by or on behalf of B4Fysics without prior written permission


Article 10. Archiving

  • B4Fysics will keep the file formed during the execution of treatments and/or services for at least 5 years after termination of its treatments and/or services and closure of the file
  • Financial data will be kept for at least 7 years


Article 11. Privacy

  • The practice complies with legal obligations regarding confidentiality and personal data guidelines
  • Your privacy is guaranteed with regard to the legal obligations of Personal Data Protection


Article 12. Complaints and Dispute Resolution

  • B4Fysics participates in the complaints procedure of the KNGF

Article 13. Applicable Law and Forum Choice

  • Dutch law exclusively applies to all agreements
  • The competent court in the district of Zutphen is also competent to hear disputes arising from agreements concluded with the patient
  • These general terms and conditions are valid from July 1, 2019, for ongoing treatments and/or services and new treatments and/or services that B4Fysics performs and/or provides
